Important: Kathleen Conlon Art will not accept partially completed documents. Since sales tax rules differ between states, we strongly encourage applicants to include under what tax provision they are requesting to be exempt as it relates to the state you are submitting. If your organization is exempt by state statute and your state does not issue a state Tax Exempt ID please make sure to note this on your official request.

Kathleen Conlon Art will accept and process tax exemption requests in Good Faith. You agree to accept all liability for any misrepresentation or misuse of any tax exempt purchases initiated with Kathleen Conlon Art. If your exemption status changes at any point, it is the customer’s responsibility to notify Kathleen Conlon Art of any and all changes before placing any further orders on any Kathleen Conlon Art accounts containing tax exempt status.

Furthermore, you agree to use your tax exempt account to only place orders that qualify for tax exempt purchases and that it is your responsibility to create a separate retailer account for orders that do not qualify as tax exempt purchases.
